You’ll usually visit several clients in a day, doing tasks like helping with personal hygiene, toileting, grooming, and mobility transfers. Expect to prepare meals or sandwiches, assist with feeding, do light housekeeping, and provide companionship and emotional support.

You’ll also monitor clients’ condition and skin, record routine observations, document care provided (using systems like Mi-Co Mi-Forms or MEDITECH), and sometimes transport clients to appointments or outings in a vehicle.

Common tools include Electronic Visit Verification (EVV) systems — like August Systems Visit Wizard or Appletree — to clock visits and confirm visit verification with GPS or a phone app. Mi-Co Mi-Forms and MEDITECH are used to document care notes, routine observations, skin checks, and feeding assistance.

You may also use FaceTime or computer reading tools for communication assistance (typing correspondence or obtaining information) and to check in with nurses or family members remotely.

You can use AI to draft or transcribe simple messages, prepare visit notes, or convert spoken notes into text, but always check accuracy against the client file. Never let AI make clinical decisions or replace your observations of skin, mobility, or feeding needs.

When storing or sending client data, follow your employer’s privacy rules and HIPAA-like regulations. If you use an AI tool, get permission and log it in your documentation system (Mi-Co, MEDITECH, or your EVV notes).

You’ll assist with mobility and transfers regularly: helping clients stand, use a walker, or move between bed and chair. You’ll also help with toileting and grooming, which can require steady, safe support. Employers usually teach specific techniques on day one.

Ask for hands-on training in transfer aids, gait belts, and how to document mobility status in systems like MEDITECH. If a client needs mechanical lifts, you must be trained and cleared to use them.

An elder care assistant focuses on daily living tasks: personal hygiene, meals, companionship, light housekeeping, and mobility help. You document observations and support autonomy but do not perform most clinical treatments.

A nurse aide or home health nurse does clinical tasks like wound care, medication administration, and clinical assessments. If clinical needs appear, you report them and document via MEDITECH or your agency’s system so a nurse can act.
