I finished a 12-hour night shift on Med-Surg and I have eight patients with new vitals, one who spiked a fever three hours ago, and two med administrations with possible minor reactions. The day nurse expects a clean chart at 0700 and the clinical coordinator audits documentation at 0730. I cannot tell if my notes are thorough enough to avoid phone calls from the oncoming team.
