Open Adobe Acrobat Pro on your computer.
Select the Tools tab.
Find and select Prepare Form under Forms.
Choose an existing file to use as your form, or select the option to create a new one.

Use the toolbar to add, move, or delete form fields such as text boxes, checkboxes, and drop-down lists.
Double-click any form field to edit its properties, such as name, tooltip, and appearance.
When finished, use File > Save to save your fillable PDF form.
You can quickly test your form by switching to Preview mode in the Prepare Form tool to see how users will fill it out.
This runs in the Acrobat app - there is no separate API for this task.
A Tooltip is a short description that shows up when someone's mouse hovers over a form field. It is very important for accessibility. Screen readers use Tooltips to tell users what each field is for. This helps people who cannot see the form clearly understand how to fill it out correctly.
By default, users of the free Adobe Reader cannot save filled forms. To let them save, you must enable special features. In Acrobat, go to 'File', then 'Save as Other', then 'Reader Extended PDF', and choose 'Enable More Tools'. This adds extra rights to your PDF, allowing Reader users to save their changes.
You should not use Microsoft Word's own form tools because they do not export well to accessible PDF fields. When you convert a Word document with Word forms to a PDF, the form fields often lose their accessibility features or do not work correctly. It's best to create your document in Word without form fields, then add all the form fields directly in Adobe Acrobat using 'Prepare a form'.
