Open your PDF file in Adobe Acrobat.
Go to the Tools pane and select Edit PDF.
In the secondary toolbar, click Header & Footer and choose Add.
In the Add Header and Footer dialog, click inside the header or footer text box where you want to insert a dynamic field.
Click Insert Page Number to add the current page number, Insert Date to add the current date, or Insert File Name to add the file name; repeat as needed.

Preview your header and footer in the dialog, then click OK to apply changes.
Save the PDF to retain the new header/footer with dynamic fields.
You can use the preview area in the header/footer dialog to see exactly how your dynamic fields will appear on each page.
This runs in the Acrobat app - there is no separate API for this task.
