Open your PDF drawing in Adobe Acrobat.
Select the Measure tool from the right pane or from the Tools menu.
In the secondary toolbar, click the Measurement Tool options (gear icon or right-click in the drawing area).
Choose Change Scale Ratio from the context menu.
In the Change Scale Ratio dialog, enter your custom scale (for example, set '1 inch = 10 feet').
Click OK to apply the new measurement scale.
After setting your custom scale, all subsequent measurements will use the real-world units you specified.
This runs in the Acrobat app - there is no separate API for this task.
A geospatial PDF is a PDF that has real-world location information built into it. This matters because when you use the Measure Tool on a geospatial PDF, Acrobat can automatically give you true ground measurements, like actual miles or meters. You do not need to set a scale manually, which makes measuring maps and plans much easier and more accurate.
If your PDF is a scanned image, it is very important to set the measurement scale correctly. Find a known distance on the drawing (like a marked wall length). Use the Measure Tool to measure that known distance on the PDF, then tell Acrobat what that distance should be in real life. This sets the scale for the whole document, making all other measurements accurate.
Your measurements are in page units because you likely have not set a measurement scale. Acrobat needs to know how the size on the page relates to real-world sizes. To fix this, you must set the scale (e.g., 1 inch = 10 feet) in the Measure Tool's settings. Once the scale is set, your measurements will show in the correct real-world units.
