Add multiple choice grid

I need a multiple choice grid added to the survey for Professor Alvarez's psych class. Create a grid titled How satisfied are you with each course element? with rows: Lectures, Readings, Assignments, Feedback and columns: Very satisfied, Somewhat satisfied, Neutral, Somewhat dissatisfied, Very dissatisfied. Make answers required and allow only one column per row.

Before I send this to the students, make the multiple choice grid easy to scan: put the satisfaction scale as the top row header, keep row labels short, set required so every row must be answered, and ensure mobile layout shows options stacked rather than tiny buttons. Flag any rows that might confuse students (for example if Lectures vs Readings overlap) and suggest clearer wording.

I created the survey and the instructor asked for a multiple choice grid to capture satisfaction by element. I can add rows and columns but worry students will skip rows because the grid looks crowded and averages will hide differences.
