Record decisions and outcomes

Capture yesterday's product roadmap decision in the decisions log, include who voted, the rationale, any dissenting concerns from Ana and Mateo, and link the related tickets and the recording. Notify the execs and legal that the decision is recorded and where to find it.

Before I add the outcome from today's architecture discussion to the decisions page, make the entry easy to use later: summarize the decision in one sentence, list the alternatives considered, the trade-offs, who needs to act next and by when, and flag any unresolved risks that would make someone revisit the decision.

In the architecture meeting we agreed to defer the migration and to run a pilot, but nobody left with an assigned owner for the pilot. I chaired the session, the architects spoke, execs listened. I'm worried the pilot will never start because ownership is unclear. Who probably should own the pilot and how do I capture and assign that responsibility clearly so the work starts this week?

Across many meetings we've been diligent about recording decisions, but these entries often lack explicit owners and deadlines, so nothing moves forward. I maintain the decisions register and stakeholders span Product, Eng and Legal. What single change in how I record outcomes will increase execution and accountability?
