Create a new survey form

I need a new survey for my 11th grade chemistry class to collect lab safety agreements and preferred lab partners. Create the form titled Safety and Partners — include a brief intro that says one response per student and deadline Friday at 3pm. Add fields for student full name, homeroom teacher (dropdown with Ms. Alvarez, Mr. Chen, Ms. Dorsey), an acknowledgement checkbox that reads I have read the lab safety rules, and a partner preference question that lets students pick up to two classmates by name. Before I share this with the 11th grade, make the new Safety and Partners form quick to finish and obvious for students. Move the deadline and instructions to the top in one sentence, mark required fields clearly, change the partner preference to a multiple-selection grid so students can pick two classmates, and add a short confirmatory message after submit that tells them to check with me if they have no partner. I set the Safety and Partners form live and now students report submitting multiple responses. I need to stop duplicates, keep current responses, and tell students what to do if they legitimately need to change their answer. What likely caused this, and what steps should I take now to lock one response per student, migrate any duplicated entries to one record, and communicate the change to homeroom teachers?
