Duplicate an existing form

I need an exact copy of last term’s Course Feedback form so I can run it for spring. Duplicate the existing Course Feedback form titled Course Feedback Fall 2025 and name the copy Course Feedback Spring 2026. Keep all questions, response settings, and the linked spreadsheet intact. Make sure the new form’s responses go to a new sheet in the same spreadsheet, and set the new form to accept responses.

Before I send the duplicated Course Feedback Spring 2026 to students, make it easier for faculty to scan results. Add a short summary section at the top that will ask students to rate overall course satisfaction on a 1–5 scale, move open comments to the end, and add a required short-answer field for the instructor name. Confirm the spreadsheet link is separate from the Fall 2025 responses so reporting won’t mix terms.

I copied Course Feedback Fall 2025 to make Course Feedback Spring 2026. Faculty fear responses will merge with Fall data. I don’t know if the new form is creating a new sheet or appending to the old one. How can I verify where responses are going, fix the destination if it’s wrong without losing submitted Spring responses, and reassure faculty quickly?
