BPT (Physiotherapy) in India — eligibility, exams, fees, careers

A professional degree that trains you to restore movement and function through exercise, manual therapy and physical methods, after injury, surgery, stroke, sports strain or long illness. Physiotherapy is now regulated under the National Commission for Allied and Healthcare Professions (NCAHP) Act, 2021, which has issued a national competency-based curriculum. It suits PCB students who want a hands-on clinical career focused on rehabilitation and are willing to build skills and a client base over time.

Know this. The course length and admission rules have just changed. UGC's August 2026 gazette notification lists B.PT as a 5-year degree (up from 4½), in line with NCAHP's national curriculum of four years plus a one-year internship, and NCAHP says candidates must have appeared in NEET-UG. States and colleges are adopting this at different speeds. Before paying, ask the college in writing which curriculum and duration your batch will follow and whether the programme is enrolled with NCAHP or the state allied health council.

Who can apply

According to NCAHP's physiotherapy curriculum: Class 12 (or equivalent, including National Open School) with at least 50% aggregate in Physics, Chemistry and Biology; you must have appeared in NEET-UG; minimum age 17 at admission; and a certificate of physical fitness. Admission is then on merit, which the NCAHP handbook bases on Class 12 marks, although universities and states apply this differently. Seats are reserved for candidates with disabilities under the RPwD Act rules described in the handbook.

How admission works

Admission practice is in transition. NCAHP's curriculum says no candidate should be admitted without having appeared in NEET-UG, with merit on Class 12 marks. In practice some state health universities admit through NEET scores in their paramedical or allied health counselling, some run their own entrance tests, some central and private universities use CUET-UG or their own test, and many private colleges admit on Class 12 marks. Government seats (for example at AIIMS or state medical colleges with physiotherapy departments) are few and competitive. Look for your state health university's allied health or paramedical counselling notice after NEET results, usually from July onwards.

Duration: 5 years (4 years of study plus a 1-year full-time rotatory internship). UGC's degree specification of 27 August 2026 raised B.PT from 4½ to 5 years; batches that started earlier may finish on the older 4½-year pattern

You like helping people improve steadily over weeks, not just a single visit
You enjoy anatomy and how the body moves
You are comfortable with physical, hands-on work with patients
You are interested in sports, fitness or rehabilitation

Think twice if

You want to prescribe medicines or perform surgery
You expect high salaried pay immediately after graduation
You dislike long hours on your feet and physically tiring work
You are taking it only as a fallback and have no interest in rehabilitation

The honest part

Starting salaries in private clinics are low for a 4½–5 year degree
Many physiotherapists build income through home visits and their own practice over several years
Regulation under NCAHP is still being set up across states; registration processes are changing
The use of 'Dr.' as a prefix by physiotherapists has been disputed; do not choose the course for the title
Demand is rising with sports, ageing and lifestyle disease, especially in cities

What it costs

Government: Government colleges charge roughly ₹15,000–1 lakh a year; seats are limited
Private: Private colleges charge roughly ₹80,000–3 lakh a year
Help: State scholarships and fee reimbursement for reserved and low-income categories generally apply. Home-visit or clinic practice later needs a modest investment in equipment.

Starting pay

Freshers commonly start at roughly ₹15,000–35,000 a month in hospitals and clinics. Income rises with an MPT, a specialisation (sports, neuro, musculoskeletal) and especially with own practice or home visits, where experienced physiotherapists in cities can earn well. Government physiotherapist posts are few.

Questions families ask

Is NEET needed for BPT?

NCAHP's physiotherapy curriculum says no candidate should be admitted without having appeared in NEET-UG. Many states and colleges are still switching to this, so check your state's notice; taking NEET keeps all options open.

How long is BPT now?

Five years. UGC's specification of degrees notified on 27 August 2026 increased B.PT from 4½ years to 5 years, and NCAHP's curriculum sets four years of study plus a one-year full-time rotatory internship. Students already in the older 4½-year pattern are likely to finish on it; check with your university.

Can BPT graduates open their own clinic?

Yes, physiotherapists commonly run independent clinics and home-visit practices. Registration and practice rules will be set under NCAHP and state councils, so keep your registration up to date.

BPT or MBBS?

They are different professions. MBBS diagnoses and treats disease with medicines and surgery; BPT restores movement and function with exercise and physical methods. If you are choosing BPT only because MBBS did not work out, think about whether rehabilitation genuinely interests you.

Can physiotherapists work abroad?

Yes, but countries such as the UK, Canada, Australia and the US require their own registration or licensing exams, and some require a longer or specific degree. The move to a five-year course is partly meant to align with international standards.
